How to Enter Surveys

There are two methods for entering surveys into Wyman Connect:

1. Teens take surveys online. Teens are provided a link to take the survey and their responses are automatically recorded in Wyman Connect.

2. Teens take surveys on paper and the data is manually entered into Wyman Connect by a designated staff person(s).

Remember that only teens with survey consent should be administered a survey.

Teens Taking Surveys Online

Using the following steps, teens can take the survey on any mobile device:

1. Share the teen-centric survey page found at: http://wymanconnect.org/SurveyTeen

   1. This allows teens to access the survey online. They do not need a Wyman Connect login; please do not share your personal Wyman Connect login information with teens.

2. Provide teens with the following information which they will be required to enter on the survey screen:

   1. Teen ID: Provide each teen with the ID that they are registered with in Wyman Connect. Share with each teen individually, do not share the list of IDs with the entire club.
   2. Club ID: Provide each teen with the Entity ID that corresponds to their Club in Wyman Connect. This can be found by searching for the Club in the Attendance or Manage Teens page(s).
   3. Survey Type: Teen Connection Project Survey

   

3. Click the Take Survey button.

   1. The survey screen opens.
   2. Teens enter responses as they appear on the survey screen.

4. After all responses are entered, click Finish. The teen will see a page confirming the submission of their survey.

   

Entering Surveys Taken on Paper

1. Under the "Data/Reports" menu, select "Surveys".

   

2. Fill in the required information in each field.

   

   Field	Description
   Teen ID	Enter the Teen ID already added to the system when the teen was registered
   Partner	Select relevant Partner name from the drop-down
   Provider	If applicable, select a Provider name from the drop-down menu
   TOP Club	Select the TOP Club the teen belongs to from the drop-down menu
   Survey Type	Select whichever TOP Survey you are administering from the drop-down menu
   Date Survey Taken	Enter date the teen completed the survey (which may differ from the date the survey is being entered)

3. Click the "Take Survey" button.

   1. The survey screen opens.
   2. Enter all responses as they appear on the survey you are entering.

   

4. After responses are entered, click "Finish".

   1. Click OK when Survey Completed message is displayed.
   2. Under the Result section, the information about the newly completed survey is displayed.

   

5. Repeat the previous steps until all paper surveys have been entered into Wyman Connect.

Accessing Survey Search History

To obtain a history of surveys that have been entered into the system (either via youth taking surveys online or manual entry of paper surveys):

1. Select "Surveys" under the "Data/Reports" menu.

   

2. Add search criteria.

   

   Field	Description
   Provider	If applicable, select a Provider name from the drop-down menu
   TOP Club	Select the TOP Club the teen belongs to from the drop-down menu
   Survey Type	Select whichever TOP Survey you are searching for from the drop down menu

3. Click "Survey Search History."

4. Results are displayed below the search form.
